I grew up in upstate New York, the son of first-generation Italian immigrants who came here with nothing but determination and an unshakeable work ethic. My father left school in eighth grade during the Depression and did masonry work his whole life building walls in brutal heat and bitter cold that would outlast him. My mother could do anything she set her mind to. Money was tight, always, but they taught me something invaluable: life was about more than money. It was about family, discipline, and doing whatever you do to the best of your ability.
I started playing music at five years old. That discipline of practice and repetition, the slow building of skill shaped how I'd approach everything else. A chance encounter at a Future Doctors of America meeting set me on a path toward surgery. I worked my way through college, earned my way into medical school, and spent five years in a general surgery residency where 100-hour weeks were the baseline.

In 1990, I opened my own plastic surgery practice. But the landscape was shifting. Managed care was squeezing harder every year, and I realized my income was tied directly to my physical presence. If I stopped working, the money stopped. But the bills didn't. That realization sent me on a decades-long exploration of real estate, financial markets, and private equity each teaching me something valuable, but also reinforcing the same lesson: everything required constant involvement.
Then I turned the microscope on my own practice. What I found shocked me. Some of our highest-revenue procedures were actually our least profitable. Revenue wasn't profitability. Being busy wasn't the same as being profitable. That was the realization that changed everything.
